All Floridians 12 years of age and older are eligible to pre-register for a vaccine appointment NOW. After registration, individuals will be notified when appointments become available in their area. All individuals under the age of 18 must complete this form and be accompanied by a guardian at the time of vaccination.

Osceola County

Registration for COVID-19 vaccinations in Osceola County will close and re-open as vaccines become available. Currently, they are only offering the COVID-19 vaccine to individuals who are 65 and older or health care personnel.

Polk County

The Florida Department of Health is providing COVID-19 vaccines to those wanting one from 11 a.m. to 6 p.m. at the following locations:

  • Mondays - Lakeland Health Department, 3241 Lakeland Hills Blvd. in Lakeland;

  • Wednesdays - Auburndale Testing location, 245 E. Lake Ave. in Auburndale;

  • Fridays - Haines City Health Department, 1700 Baker Ave. E. in Haines City.

The Florida Department of Health in Polk will offer the Moderna and the Johnson & Johnson vaccines. No appointment is necessary, however, if individuals wish to make an appointment, they may contact the DOH Polk Information Center at (863) 519-7911.

Vaccine is also available at 54 locations in Polk County, through both federal and state programs. Below is a list of federal programs and local partners:

  • Winter Haven Hospital;

  • Watson Clinic, Bond Clinic, Gessler Clinic, Ridge Medical Group and Health Medical Ultra;

Florida has many sites that are providing COVID-19 vaccines to eligible persons. Please be aware that some locations are only serving very specific populations. Appointments may be required and vaccine availability will vary from day to day and week to week, as we work to provide vaccines to the most vulnerable first.
